If you have been told there is not enough bone

This is the most common reason a Dallas patient ends up in our chair. An upper jaw that has carried a denture for years resorbs, the sinuses drop, and a practice measuring for standard implants finds nothing to anchor into. The usual answer is a bone graft and a sinus lift, then nine to twelve months of waiting, then implants — if the graft takes.

There is another route. Zygomatic implants anchor into the cheekbone rather than the upper jaw. The zygoma is dense, it does not resorb the way the alveolar ridge does, and it is available immediately. Pterygoid implants do something similar further back, anchoring into the pterygoid plate. Together they make fixed teeth possible in an upper jaw that cannot hold conventional implants, often without grafting and often on a much shorter timeline.

These are not routine procedures and we will not pretend otherwise. They demand a surgeon who places them regularly, three-dimensional planning, and honest discussion of what can go wrong. Every figure is all-inclusive on the same basis: the surgery, the implants, the abutments, the provisional, the final prosthesis and the warranty. Zygomatic or pterygoid work on an upper arch adds $10,000.

Why Dallas patients drive north

Dallas has no shortage of implant providers. What is harder to find is a practice that does only this.

We place implants and nothing else. There is no hygiene department subsidising the surgical schedule and no general dentistry competing for chair time. The laboratory is in the building, which is why a provisional can be delivered the day after surgery rather than sent out and waited on, and why a chipped crown on a Malo Bridge is replaced individually instead of the whole arch being remade.

The other reason is the number. Full-arch treatment across the wider Dallas–Fort Worth market commonly quotes from the mid-twenties into the mid-forties per arch, and what is included varies so much that two quotes are often not describing the same work. Ours is published, all-inclusive, and does not move between the consultation and the surgery.

Getting here from Dallas

We are at 4708 W Plano Pkwy, Suite 200, just off the Dallas North Tollway. From downtown Dallas it is a straight run north on the Tollway, usually around 25–35 minutes outside rush hour. From North Dallas, Preston Hollow or Addison it is considerably less. Parking is free and directly outside the suite — no garage, no ticket, which matters on a sedation day when someone else is driving you home.

Grafting or zygomatic: how the decision actually gets made

Both are legitimate. Which one applies to you is a question about bone, not about preference, and it is settled by a scan rather than a conversation.

Bone grafting and sinus lifting rebuild the ridge so that conventional implants have somewhere to go. It is well-established, predictable in the right case, and it is what we recommend when there is a reasonable foundation to build on. The cost is time: graft, then heal, then implant, then heal again. Nine to twelve months is normal and it can be longer. Grafts can also fail, more often in smokers and in poorly controlled diabetes, and a failed graft means starting over.

Zygomatic implants skip the deficient bone entirely. Instead of rebuilding the upper jaw, the implant is angled up into the zygomatic bone — the cheekbone — which is dense, stable and rarely affected by the resorption that destroyed the ridge. Because there is nothing to heal before placement, a provisional bridge can usually go on within a day or two rather than the following year.

The honest trade-off is that zygomatic placement is technically demanding and the complications, while uncommon, are different in kind — sinus involvement chief among them. It is not a procedure to have done by someone who places two a year. It is also not a procedure to be talked out of if the alternative is a year of grafting with an uncertain outcome.

What we will do at the consultation is take our own CBCT, measure what is actually there, and tell you which of these your anatomy supports. If a graft is the better answer for you, that is what we will say, and it is what we will quote.

What the surgical day looks like

Full-arch surgery, zygomatic or conventional, is done in one appointment under IV sedation. A single arch runs about two hours; both arches, four to six. You are asleep for it and you go home the same day with someone else driving.

Fixed provisional teeth go in immediately or the following day — they are milled in our own laboratory rather than sent out, which is the whole reason the timeline works. Those provisionals are working models, not the finished result: the bite is adjusted over several visits, speech settles within a few weeks, and the gums change shape as they heal. The final prosthesis is made once that has stabilised.

Expect swelling for several days and a soft diet for a few weeks. Most people are back at desk work within a week. The patient journey page sets out the full sequence.
